Program Transition Intent Form

By submitting this form, I acknowledge and agree to the following:

• I understand that ACEE BRIDGES Support Services will conclude its operations on December 31, 2025, due to organizational and contractual changes at the corporate level.

• I have reviewed the two available options and have selected my preferred transition path:

  1. Assume the Lease directly with Morgan Properties – The Marylander Apartments,

  2. Vacate by December 15, 2025, or

  3. Continue Housing with ACEE Covering Rent Only

• I understand that, regardless of my selection, I will receive a prorated refund for the unused portion of my housing and support services through December 31, 2025.

• I understand that if I choose Option 1, to assume the lease, I will be able to keep all of the furnishings that was provided by ACEE, at no extra cost.

• I acknowledge that prorated refunds will be processed within 45 days of my confirmed transition date and that earlier completion of the transition may result in earlier refund processing.

• I understand that this form serves as my official notice of intent, and that my selection below is considered final once submitted, unless otherwise approved by ACEE in writing.

• I agree to cooperate with ACEE staff to complete the transition process, including move-out coordination or lease transfer, as applicable.
